Introduction

The Questionnaire page allows you to manage and assess questionnaires for a third-party.

Questionnaire List

The list displays all applicable questionnaires for the selected third-party, based on its defined tier.

For each questionnaire, you can view:

  • Name

  • Assessment Source

  • Number of Questions

  • Progress

  • Last Updated

From this list, you can also send the Assessment Request to the vendor contact.

Requesting an Assessment

  1. Locate the questionnaire in the list.

  2. Click the Send Assessment Request option.

  3. Enter the Primary Contact Email.

  4. Set the target due date.

  5. Click the Send Request button. The system sends an email to given primary contact email with a link to assess the questionnaier.

Manage Questionnaires

You can add or remove questionnaires for a third-party.

To manage questionnaires:

  1. Click the three-dot menu at the header row of the list.

  2. Select Manage Questionnaire.

  3. In the pop-up, select the questionnaires you want to associate with the third-party.

  4. Click Save to apply the changes.

Assess a Questionnaire

To assess a questionnaire:

  1. Click on a Questionnaire from the list.

  2. You will be taken to the Questionnaire Assessment page. Here you can view the Assessment Summary, which includes:

    1. Total number of questions

    2. Number of answered and pending questions

    3. Percentage completion

    4. Overview

    5. Last interaction with the questionnaire

  3. Uploading an Assessment:

    1. You can upload a completed questionnaire file.

    2. Based on the uploaded file, SAFE will automatically assess the questionnaire.

  4. Answering Questions Manually:

    1. Scroll down to the questionnaire list.

    2. Read each question, select the answer, and (optionally) add a rationale.

    3. Attach supporting evidence if needed.

  5. Question Grouping:

    1. The system groups questions based on their nature.

    2. Click the three-line (group) icon to view the grouping.

    3. Groups allow you to quickly navigate between related sets of questions.
